Job Description



The Medical Records Coder is responsible for abstracting and coding Inpatient Unit medical records using ICD-10-CM diagnosis codes; assist with outpatient medical record coding as needed.; audit charts for documentation improvement and report inefficiencies to the Medical Records Manager.


  • Abstract and code Inpatient Unit termination summary's upon discharge using ICD-10-CM diagnosis codes
  • Code physician supernotes using CPT codes
  • Review charts for documentation to support diagnosis(s)
  • Report deficiencies in documentation to Medical Records Manager
  • Cross audit outpatient charts for coding accuracy
  • Collaborate with team members and assist with outpatient coding when needed
  • Assist with designing and teaching audit techniques to staff to proactively identify documentation problems and report them to the appropriate manager


Job Requirements

Education:  High School Diploma or GED Required; Associates degree preferred.

Certificate or Professional License: AHIMA Certification as a CCS, CCS-P or AAPC certification as a CPC, CIC, or COC. 

Experience: Prefer two (2) years in a mental health setting and (1) year experience in ICD-10, CPT  and Emergency and Management Coding
