The skills coach is responsible for providing case management and skill building services to an assigned caseload. As a Skills Coach, you will provide services at Bowen Center locations, in the community, and in client's homes. Your caseload may consist of child/adolescent or adult population depending on the location and the need. Duties of this role may include:

• Facilitate clients' learning and development of new skills, improving functionality and independence
• Ensure clients are linked with other needed healthcare providers
• Ensure clients are addressing the goals that are important to them and/or their referral source
• Participate in medical appointments with the organization’s medical staff
• Ensure clients are making progress and are discharged within an appropriate timeframe
• Transport clients as needed and appropriate to their care

$13.00 - $14.50/hour
